Satisfactory Academic Progress Quiz
Students must complete ALL questions. Students are considered to have met the requirements of passing the quiz with an 80% (or better).  Notification of "pass or fail" will be sent via email within 3 working days of completion.  WE MUST HAVE AN EMAIL ADDRESS ON FILE IN ORDER FOR US TO INFORM YOU OF YOUR RESULTS.  Update your email with Admissions & Records if applicable.

The email will inform you of one of the following:

1. If you failed this online SAP workshop, you may redo the SAP online workshop for a second time;

2. If you failed the SAP online workshop twice, you must attend a SAP workshop in person, or

3. If you passed, you will be informed that your FA Appeals form is being emailed as an attachment.

There are two opportunities to take and pass the SAP quiz during the term of disqualification. The deadline to appeal for each term is final. You may not appeal once the deadline for the term has passed.

If you are on probation, please DO NOT submit this quiz.

8. What is Return to Title IV (R2T4)?
   A Federal program requiring all students to return to school.

   Federal Regulation implemented by law requiring all institutions of higher education to calculate the percentage of completion of courses of students who drop ALL their courses.

You may only take this quiz on-line twice per semester. The third attempt must be taken at a SAP workshop in person. Once you pass your quiz it is valid until the following Fall semester.
