EOPS/CARE APPLICATION
By completing this application you may be eligible to receive EOPS/CARE services. EOPS/CARE applications are accepted every semester (Fall, Spring and Summer). You may download the application, complete it and then take it to the EOPS/CARE office.

MIDTERM PROGRESS REPORT
The Midterm Progress Report for SPRING 2010 is due on April 16, 2010. Note for Internet classes: an email from the instructor evaluating grade, attendance and academic effort is to be sent directly to an EOPS/CARE counselor. Students: “cc” an EOPS/CARE counselor in the email when requesting evaluation from instructors.

EOPS/CARE EXIT SURVEY
This exit survey is intended for students who are no longer eligible to receive EOPS/CARE services. If you are a current EOPS/CARE student and would like to offer feedback, please download this anonymous form, complete it and submit it to the EOPS/CARE office.
