• This form must be sent by the Office of Student Life in order to be processed.
• Wait for review by the Office of Student Life.
• Once approved, complete your Club Orientation.
• Clubs are not considered fully ratified until the registration form is completed and orientation has been attended. No exceptions

ICC Representatives must attend bi-weekly ICC meetings. All active clubs must have an ICC Representative present at all ICC meetings. 

NOTE: A club’s charter shall be revoked if the club’s ICC representative has three unexcused absences in the academic year. Excused absences shall be determined by the ASHC advisor.
